“Accepted” means that Centenary Family & Urgent Care has agreed to provide care to the customers of a particular insurance program. We would like to be able to participate in all insurance programs; however, this is a complex decision which considers the availability of hospitals, specialists, referral restrictions, and drug formularies.
Please bring your insurance card to every visit!
Please note: Specific benefits and co-payments are determined by your plan.
If we participate in your health plan, we will file a claim with your insurance carrier for the services provided. However, if your plan requires you to pay a co-payment or deductible, this will be due at the time of service.
If we do not participate with your health plan, full payment for your services will be collected at the time of your visit. As a courtesy, for most insurance companies we will file the claim to your insurance carrier and they will reimburse you directly. Please see our Finical Policy for additional details.
